Follow these three steps to submit your Client's funding application:
Fill out and submit the form on this page.
Email the Client Summary Sheet to firstname.lastname@example.org.
Upload the following supporting documents to Box.com:
- Financial Statements (Income Statements and Balance Sheet for last fiscal year-end and year-to-date)
- Bank Statements from previous two months (All accounts reflected on Balance Sheet)
- Ownership Structure (Cap table with names)
- Complete tax returns as filed for the last two years (T2 & T661)
- Notice of Assessments for the last two years (Reflecting refund received)
- Accrued SR&ED refund support and/or Grant support (Contribution Agreement, Year-to-Date snapshot)
